Free and Reduced Lunches:

Free and Reduced applications are available via the board office or print the form from your home computer and return to the Madison Board Office. Filling out these forms ensures your student(s) will have free or reduced price meals on the first day of school. You must use the current school year form. Parents are responsible for their students lunches until they receive notification that the application has been processed. Click HERE to download this year's form.

Forgotten Lunch Money

Dear Parent or Guardian:

We are pleased to inform you that the Madison Local School District will implement a new option available for the 2016-2017 school year.  Great news for you and your students!  All Madison students are eligible to receive a free healthy breakfast at no charge to your household for the 2016-2017 school year.

Under the new program, federal reimbursements are expected to increase.  This means that, even though the school will not collect money from students for breakfast, total meal revenue will increase with added participation.

Parents or guardians do not have to fill out an application for the breakfast program.

Please note that parents or guardians must still fill out a confidential meal application for free or reduces lunches.  Applications can be found at

Madison Local Schools are among 660 schools in Ohio implementing this redesigned school breakfast program.  We hope you will encourage your student(s) to participate in this program.

Research shows school breakfast is linked to increased attendance and improved academic performance.  School breakfast also provides the vitamins and minerals students need to grow healthy and strong.  School breakfast is linked to healthy weights and helps children make nutritious choices.
